Updates on Royal College Recognition of GIM

Dear Colleagues,

Further to my e-mail of June 12 regarding the Royal College Committee on Specialties decision on GIM recognition, attached please find a briefing document on the GIM application which has been circulated to involved parties at the Royal College and the Committee on Specialties in preparation for their upcoming deliberations on October 28-29 regarding our application to have GIM officially recognized as a subspecialty of Internal Medicine by the Royal College.

I am circulating this to all members of the CSIM in the hopes that you will be able to discuss this with any of your colleagues from outside GIM who are looking for more information prior to the Specialty Committee Meeting in October. I know the various parties at the Royal College have heard the cries of protest from the GIM community loud and clear over the past few months since the May decision of the Committee on Specialties to defer our application, and they have worked with Brian, me, and the various GIM Program Directors to help position our application for maximal chance of success and we are hopeful this short briefing document will be another key piece of the strategy to achieve our goal of official Royal College recognition of GIM.

To paraphrase Brian O'Brien's e-mail to me earlier today: "Once more unto the breach, dear friends, once more....."

Sincerely,
Dr. Finlay McAlister
CSIM President

The CSIM is promoting a national dialogue to promote the recognition of General Internal Medicine (GIM) as a medical specialty. We believe that the discipline of GIM is a core player in the training of medical students and residents, in research, in health promotion, in administration, and most importantly in patient care. As such it is a part of the backbone of the Canadian health care system. Assuring training standards for General Internists is of fundamental concern if they are to fulfill their role.

The Royal College is the premier accreditor of medical training in Canada, and in 2005, they formally recognized the existence of GIM, by granting us a temporary "distinct status". They gave us two years to propose an acceptable definition and training proposal for GIM. We did not succeed in that task, and although GIM has a presence in every university and training program in Canada, a national specialty society, a national peer review journal, a web site, and a national meeting, we do not exist according to the Royal College. This is unfortunate as it creates a barrier to recruitment to the discipline of GIM, undermines efforts to create national training standards, and ultimately weakens the Canadian health care system.

Recognizing the urgency of this situation, the CSIM is engaged in an active dialogue with the Royal College, and is committed to promoting a dialogue amongst Canadian General Internists that will lead to recognition of GIM. This dialogue has been under way for over a decade and has intensified progressively over the past few years. The attached PDF summarizes the current consensus that has evolved as a part of this extensive dialogue. We seek to expand this dialogue and invite the Canadian community of GIM to submit your feedback on this document through the feedback link below.

Many thanks
